Third Annual Jim Woodman Invitational 4 on 4 Coed Street Hockey Tourney SILENT AUCTION, 50/50 DRAW, BBQ, PRIZES & TUNES ALL DAY When: 9:00 a.m. – 5:00 p.m. Saturday, June 9, 2012 Rain Date is June 10th, 2012 Where: Norberry Glenlee Community Centre 176 Worthington Ave (off St. Anne’s Rd.) Cost: $160.00 entry fee per team (includes Jim Woodman Memorial T-shirt per team member & team photo) * Each participant must sign a waiver form and must be 17 years of age or older. If 17 must have a guardian or parent sign a waiver form. A maximum of 8 players are allowed per team entry. No alcohol allowed on premises. Washrooms and water will be provided. We would like to raise as much money as we can for Cancer Care Manitoba, we need your help with getting pledges. * Team waiver forms and pledges can be arranged for pick up by calling 296-5785 or email Cole Woodman at cole_woody @hotmail.com * Team entry fees, pledges and waiver forms are due at 122 Augusta Drive by: Saturday, May 19th at 5 p.m. Monies raised will be donated to Cancer Care Manitoba in aid of colon cancer research. May 1, 2012 Dear Firefighter Friends & Family: During the past two years I organized a street hockey tournament in memory of my father, Captain Jim Woodman (Reg #962) who passed away at age 51 in 2004 from job-related colon cancer as a firefighter with the city of Winnipeg. My dad gave his life to protect the citizens of Winnipeg and I want to give back to the community by holding this event again this year in his honour and raise funds for colon cancer research. All funds raised will go to Cancer Care Manitoba Foundation.
Over the past two years we have raised over $5700 towards colon cancer research in Manitoba. We would like to make this year’s event even better. We will be holding a silent auction, having a BBQ and 50/50 draw, as well as having individuals get pledges for the tournament. We are accepting donations/prizes and or pledges payable to Cancer Care Manitoba Foundation. Cancer Care Manitoba will provide a tax receipt for any donations received provided an invoice is received with the prize/donation and will also provide a receipt for any monetary donation of $15.00 or more. We would very much appreciate your support for this event! If you would like to volunteer in some way- that would be awesome too! We need referees, help with the BBQ, etc. It will be a fun-filled day! If you can help in any way, please call me or email me. We would love to have some firefighters playing with us!!! We are holding the tournament this year on June 9th, 2012 (rain date June 10th) at Norberry Glenlee Community Centre, 176 Worthington Avenue. We still have room for a few more teams. Teams can have up to 8 co-ed players. 2012 4 on 4 Jim Woodman Invitational Street Hockey Tournament Waiver Form and Roster Submission
WAIVER: In no way is Norberry Glenlee Community Centre or Third Annual Jim Woodman Invitational 4 on 4 Street Hockey Tournament liable for any injury that should occur to participate during this event on June 9th, 2012 ( rain date June 10th, 2012). All participants or parent/guardian if 17 must sign this waiver to recognize these terms prior to playing in this tournament. COST: $160.00 PER TEAM ENTRY (* includes Jim Woodman Memorial White T-shirt) PLEASE INDICATE T-SHIRT SIZE REQUIRED and also will include a team photo. Please bring a Dark T-shirt as well for Away Team designation. PLEDGES FOR CANCERCARE: WE WOULD LIKE EACH TEAM TO RAISE A MINIMUM OF $100 IN PLEDGES. CANCER CARE WILL ISSUE AN INCOME TAX RECEIPT FOR DONATIONS OF $15.00 OR MORE. A prize will be given to the individual who raises the highest dollar amount for pledges.
